What Is HUD-VASH?

HUD-VASH is a partnership between the U.S. Department of Housing and Urban Development and the VA. It pairs a permanent Housing Choice Voucher with hands-on VA case management — so a Veteran isn't just housed, they're supported for as long as it takes to stay housed.

It does not determine VA enrollment, HUD-VASH eligibility, chronic homelessness, HUD homeless category, voucher eligibility, PHA admission, or housing placement. Preliminary HUD Homelessness Screening

Select the category that appears to best describe the Veteran’s current situation. Final homeless-status determination requires current HUD/Continuum of Care definitions and required documentation.

  • Category 1 — Literally Homeless: Lacks a fixed, regular, and adequate nighttime residence; for example, sleeping outside, in a vehicle, in a place not meant for human habitation, in an emergency shelter, or exiting an institution after 90 days or less when coming from shelter or an unsheltered location.
  • Category 2 — Imminent Risk of Homelessness: Will lose the primary nighttime residence within 14 days, has no other residence identified, and lacks the resources or support network to obtain other permanent housing.
  • Category 3 — Homeless Under Other Federal Statutes: Generally applies to certain unaccompanied youth under 25 or families with children/youth meeting another listed federal homelessness definition, with persistent housing instability and barriers to stable permanent housing.
  • Category 4 — Fleeing or Attempting to Flee Violence: Fleeing or attempting to flee domestic violence, dating violence, sexual assault, stalking, trafficking, or related dangerous conditions; has no other residence; and lacks resources or support to obtain other permanent housing.
  • Unknown: Use when information or documentation is not yet available. Record follow-up steps and do not guess.

Important: Do not use this preliminary screen to exclude a Veteran from referral, services, or individualized review. HUD-VASH, VA, the Public Housing Agency, and the local CoC determine requirements and documentation.

Important: This authorization is for Heaven-Sent to coordinate housing, HUD-VASH referral, benefits, and related support. It is voluntary and does not itself determine HUD-VASH eligibility, VA enrollment, voucher issuance, or housing placement.

Limited authorization. I authorize Heaven-Sent to communicate with the person(s) or organization(s) identified below only for the purpose(s) I select. Heaven-Sent may request, receive, and disclose only the minimum information reasonably necessary for those purposes.

Voluntary and revocable. I understand this authorization is voluntary. I may refuse to sign it and may revoke it in writing at any time, except to the extent Heaven-Sent or another party has already acted in reliance on it. Refusal or revocation may affect Heaven-Sent’s ability to coordinate the particular services requested, but does not automatically prevent me from requesting other services.
