HEAVEN SENT COMMUNITY SERVICES & VETERANS ASSISTANCE

HOMELESS
SERVICES

Outreach That Finds People.
Casework That Connects People.
A Pathway Designed to Move People Forward.

Heaven Sent meets people where they are — on the street, in encampments, living in vehicles, temporarily displaced, facing housing instability or trying to rebuild after homelessness.

Our goal is to strengthen street outreach, case management, housing navigation, hygiene, feeding and coordinated-entry/HMIS information while connecting those services to transportation, healthcare resources, employment, benefits, disability services, Veteran programs, family support and long-term housing stability.

Start With Heaven Sent.

You do not need to know the name of the program before calling. Tell us what is happening and we can help identify the next responsible step.

DOCUMENTED CENTRAL FLORIDA NEED

Homelessness Is Not One Problem.
It Requires a Connected Response.

The 2026 Point-in-Time Count reported 2,724 people experiencing homelessness across Orange, Osceola and Seminole counties. The PIT Count is a one-night estimate and is generally understood to undercount the full number of people experiencing homelessness.

WHY STREET OUTREACH MATTERS

The People Most Disconnected From Services Are Often the People We Must Go Find.

In the 2026 regional count, 993 people were found living on streets, in woods, in vehicles or other unsheltered locations. Effective outreach cannot depend solely on someone reaching an office.

THE HEAVEN SENT RESPONSE MODEL

From First Contact to Greater Stability.

A meal, shower or outreach visit can be the beginning of a much larger pathway. Heaven Sent's model is designed to connect immediate human needs with casework, coordinated systems and long-term opportunities.

1
STREET OUTREACH Meet people where they are and begin building trust.
2
IMMEDIATE NEEDS Food, hygiene, clothing, water, essential supplies and safety.
3
CLIENT CASEWORK Understand the situation, barriers, documents and priorities.
4
HMIS / COORDINATED ENTRY Provide appropriate information and connect to the local homeless response system.
5
HOUSING NAVIGATION Identify appropriate housing and homelessness-prevention pathways.
6
HEALTH & BENEFITS Connect with qualified healthcare, disability, benefit and community resources.
7
TRANSPORTATION Address mobility barriers that can prevent successful referrals.
8
EMPLOYMENT Career readiness, training, apprenticeship and workforce pathways.
9
HOUSING Connection to appropriate housing resources and responsible providers.
10
FOLLOW-THROUGH Continue appropriate casework and resource coordination toward stability.
STRENGTHEN STREET OUTREACH, CASE MANAGEMENT, HOUSING NAVIGATION, HYGIENE, FEEDING AND COORDINATED-ENTRY/HMIS INFORMATION. Then connect those services to the systems and opportunities that can help a person move forward.
INTEGRATED HOMELESS SERVICES

Not One Program. A Connected Network of Support.

Heaven Sent's homeless-services model recognizes that the barrier keeping someone outside may be housing, transportation, disability, employment, documentation, food insecurity, health, family circumstances or several of these at the same time.

STREET OUTREACH AS THE FRONT DOOR

We Cannot Help People We Never Reach.

Outreach creates an opportunity to move beyond a one-time handout. Every respectful contact can become a bridge to casework, coordinated-entry information, healthcare, housing or another appropriate resource.

Locate & Engage Build consistent, respectful relationships with people living unsheltered.
Meet Immediate Needs Food, water, hygiene, clothing and essential supplies can create a safe point of engagement.
Identify Barriers Housing, disability, transportation, documentation, health, employment and family needs.
Begin Casework Move from crisis contact toward an organized plan.
Connect the System Provide appropriate coordinated-entry/HMIS information and connect with responsible community systems.
Follow Through Help reduce the chance that a person disappears between referrals.
COORDINATED RESPONSE

HMIS & Coordinated Entry Matter Because No Organization Can Do This Alone.

Heaven Sent supports stronger coordination with the local homeless response system so outreach and casework can connect people to appropriate community pathways rather than creating another isolated door.

Outreach-to-System Connection

A practical pathway for turning a street contact into coordinated action.

ENGAGE Street, vehicle, encampment or community contact.
ASSESS NEED Understand the immediate situation and barriers.
INFORM Explain appropriate coordinated-entry/HMIS pathways.
CONNECT Link with the responsible system, provider or access point.
FOLLOW UP Continue appropriate Heaven Sent casework and resource coordination.
HMIS participation, data entry, coordinated-entry assessments, prioritization and referrals are governed by applicable Continuum of Care, HMIS and provider policies. Heaven Sent performs only those functions for which it is authorized and trained.
WHO WE WALK BESIDE

Homelessness Does Not Look the Same for Everyone.

Heaven Sent's mission reaches beyond a single population. Services and referrals are shaped around the person's circumstances and the eligibility rules of the responsible programs.

People living on streets, in woods, encampments and other places not intended for habitation.

OUR STANDARD

DIGNITY IS NOT AN EXTRA SERVICE.
IT IS THE STANDARD.

Being homeless does not make someone less deserving of privacy, respect, safe food, clean clothing, appropriate hygiene, reliable information, accessible services or the opportunity to rebuild.

Heaven Sent's standard is simple: assistance should be safe, clean, usable, appropriate and dignity-preserving.

PUBLIC • PRIVATE • PHILANTHROPIC INVESTMENT

One Mission. Multiple Responsible Funding Pathways.

Heaven Sent is building a diversified funding strategy because homelessness crosses housing, health, nutrition, employment, transportation, disability, Veteran services, disaster resilience and community development.

An Investable Homeless Response System

Funding can strengthen different parts of the continuum: outreach finds people; hygiene and food support engagement; case management organizes the plan; coordinated systems connect resources; housing navigation moves toward placement; workforce services help strengthen long-term stability. Each funding source is pursued only for eligible activities under its applicable rules.

U.S. Department of Housing & Urban Development
COC • ESG • HOUSING & HOMELESS SERVICES

Potential alignment can include eligible street outreach, supportive services, case management, coordinated-entry/HMIS functions, transitional housing, rapid rehousing and permanent housing activities depending on the specific program, competition, project type and approved budget.

HEAVEN SENT + SAFE HARBOR

Heaven Sent Goes to the People.
Safe Harbor Gives People a Place to Move Forward.

Heaven Sent's work does not begin at the gates of a future campus. Street outreach, feeding, casework, Veteran assistance, housing navigation, transportation, disaster response, senior/disability support and community services happen in the community.

Safe Harbor is the next step — Heaven Sent's flagship integrated program being developed to bring housing, workforce development, supportive services, food and nutrition, transportation, technology, disaster preparedness and pathways to employment together within one coordinated model.
